As a person with a disability, you are able to choose the services you use and share how you want to be supported. Support Coordinators help you share information about yourself, your hopes and dreams, and the support you need through a person-centered planning process. They also help you select the service providers that will support you to be involved in your community and reach your goals. It’s important for you to learn more about what to look for and what to expect when working with a Support Coordinator and selecting services.
