X-WR-CALDESC:Supporting families to care for their children at home and in
natural community settings is a central goal of the NJ Children’s System o
f Care (CSOC). Sometimes youth need to be placed outside the home to rece
ive treatment in a hospital or residential treatment program. While a maj
ority of youth initially return to their home\, many begin a cycle of repe
ated hospitalizations or out of home placements. This training will provi
de information about planning for those placements and subsequent return h
ome\, even before the youth is placed. Emphasis will be placed on underst
anding program criteria for discharge\, and the need to identify skills to
be taught\, and supports that increase the likelihood of a successful ret
urn to the home and community. Strategies presented will include developm
ent of visitation schedules\, facilitating generalization of the improved
behavior the youth has demonstrated during their placement\, and making ac
commodations to the home environment and support services
